No text entry field on survey slide... HELP

I have created a one slide, one question survey slide and inserted it in my StoryLine project. I think (because I can find no way to verify) that I have it linked to a pre-existing results slide. My main problem, though, is this. If I preview just the survey slide, I get a text entry field. If I preview the entire project or even the scene that the survey slide is a part of, when I get to the survey slide, I do not get a text entry field. There is nowhere or no way to enter text into the slide.
So... The survey slide, by itself, works. But the same slide does not work in the project.

Thank you Wendy. I have attached a copy of the .story in question. The slide in question is the last slide in the project, 3.3. When I open the .story and preview this single slide, I get a text entry field. But, when I run the whole thing, or just the last scene, I get NO TEXT ENTRY FIELD. I can't see anything in the previous slides that would cause this. Desperately seeking solution. Already past deadline...

I have attempted to attach a zipped copy of my .story file but evidently it is still to large for our corp email.
I have, however, uploaded a copy to the community.articulate site in reference to your offer for assistance.

Hi Mac,
Thanks for sharing your file here. I took a look and saw the same behavior as you mentioned, but once I imported the project into a new Storyline file it behaved normally. You'll want to confirm that you're working on your project files as described here, as those are known causes of odd or erratic behavior. Here are the directions on how to import your project.
I have tried to import the current project into a new Storyline file.
The new .story has no audio.
I've gone over the steps and can find no reason for this.
I'm sure it's some box checked or unchecked... Some little something.
Any advise?
I found the problem with the audio. It was a PC problem. Nothing to do with StoryLine.
