Are you trying to add an image directly to your slide master using the placeholder?
Placeholders are designed to leave space for those items when you add them to an individual slide that is using that slide master. If you want to add an image to a slide master so that it'll appear on each slide using it, no need to use a placeholder, just add that image directly onto the slide master.
