I have set up triggers to limit the number of attempts a learner can take on a test to 3. When I preview in SL I am correctly locked out after 3 incorrect attempts however when this learning has been published and loaded in to our LMS learners are able to access the test questions again after three incorrect attempts - does anyone know why this could be.

Also, have you done any testing in SCORM Cloud yet? If so, are you getting the same results? We generally compare the behavior in SCORM Cloud to your LMS experience to try to parse out file issues versus LMS issues. We are happy to help you with that testing if you'd like to share your file!
