I'm working on a training module and I would like to use a survey as a pre-assessment with five questions. However, I would like it to all be visible at the same time on one slide. Is there a way to accomplish this task so that I could use the results to compare to the post-assessment results.

Hello Tara, and welcome to Heroes!
By default, Storyline does not support evaluating more than onedata entry fieldon the same slide. However, you may be able to evaluate multiple fields on the same slide by disguising aFreeform Pick Onequestion as demonstrated in this video tutorial.
Here is one example about doing a pre/post assessment form the community.
Hopefully others may chime in with some other ideas as well!
