We have a troubleshooting document here for the common reasons that you may experience this.
If that doesn't fit the bill, I'd be curious if you are able to replicate this in SCORM Cloud or if the issue is limited to your LMS. If you're able to replicate, we would certainly want to take a look at the .story file.
