video quality after publishing

I've seen several threads about this but nothing seems to be fixing my problem. I'm going crazy trying to get good video quality when I publish. The material I'm using as a source is a high quality studio master, either DVD or mp4 version. When I preview the video in the project, it looks wonderful but after I publish to either CD or web, it looks horrible (see link below).
I know that part of the problem is that I have my publish setting to scale to fit browser window, but the project looks fantastic large and it's a lot of material for usersto view in a smaller screen. I've attached all the ways I've tried to resolve the issue, none of which have worked. Please let me know if I shthere's anything possible to fix it because I can't have it look like this. http://solutions-unlimited.co.nz/video%20problems%20output/story.html
(PS--not to worry, the therapy session isn't real clients, but actors, so no confidentiality problems!)

Just curious, what are you currently using for the publish quality settings?
To check, go to Publish>CD and click on the blue text to the right of "Quality".

Wow--never even knew those settings existed. Will try to republish right now with the higher video quality. Thank you.
Just re-published what I sent you and two out of the three look great at the high quality setting for video. Learning something useful every day. You're a treasure!
