weird stuff happening when I rerun a scene

Can anyone help me to understand what I'm doing wrong here... I've created three lessons, all quite complex, and they work ok apart from replaying any of the elements.
There are mini quizzes at the end of each section for example, which work fine. if you go back to the quiz though, it doesn't reset itself to it's original state, and allow you to retry it. Next and previous buttons appear from nowhere, and there is no facility to submit an answer. It seems to be remembering the answers previously given. It's the same in the learning sections, if they go back over those, and there are places where text has been entered for example, the text is there again when you go back.
I've tried setting the slides to say 'when revisiting, reset to initial state', but I'm at a loss as to what else to do. I've ripped a small section out of one of the lessons to poke, and see if I can work it out, and I've attached that here, it's just a four question quiz. Any advice would be gratefully received!!!

Hi, Paul -- Thanks so much for reaching out here and sorry to hear of your troubles! I tested your course via the SCORM Cloud and I am finding the same behavior you've reported. I also checked your resume settings and confirmed that you have your question slides are set to Resume Initial State, but an option to submit never appears and there are instances where I was unable to even select an answer choice. I'd like to have our Support Engineers take a deeper look, so I will go ahead and set you up in a case momentarily, and I'll be sure to follow along with their progress.

I've discovered the 'Reset Result' trigger, which has cured my woes with the quizzes, but not resolved the same issue which is occurring during the lesson element, which is a software simulation, so users enter data into fields, and on revisiting, the data is still there, or radio buttons are selected etc, and they can't move on.... I'm in discussion with support now. :-)

Hi Kim, it turns out that the data fields in the lesson elements were being treated as questions, so removing the freeform question has stopped this. In the quiz elements, you need to add the reset results trigger on the score screen, when the user clicks to go back over the quiz.
