wrong answer options appear in Word

Apologies if this is a well-worn topic, but the last few times I've published a Storyline quiz in Word, some correct answer options are shown incorrectly, both in the screen image and in the text underneath. This is a problem because I usually supply the quizzes in Word to my clients for review, and - of course - they point out that the answers are wrong. This happened once or twice in earlier quizzes, but has been happening on more questions in the last couple of days.

Could it be because I'm duplicating questions and then amending the duplicates?

I'm also getting lots of 'low memory' warnings.
