Other community members may have additional ideas, but the first option that came to mind for me was to create a drag and drop activity for this. You would pre-create a number of lines and circles that students could choose from to annotate the image.
That said, if you're looking for an interaction that's quite quick and easy to create in Storyline, another idea could be to change this to a multiple-choice question. Present students with a few annotated images and ask them to choose the one that's correct (or most accurate if this is a topic that could have multiple correct options).
