HELP: Convert to freeform and Result slide

Hello friends,

I have watched a tutorial by David on converting to freeform for multiple questions on a slide, it has helped greatly but I'm still stuck, please help me out. Here is a gif of what I've done...

I have already created button sets for each question. I have also asked the result to score 'by choice' so that I can assign a point to each choice and have it calculate the final score depending on what choice was chosen.

1. It is, however, not submitting my results in the result slide?
2. I actually want it to tell me what point the user chose for each question (each button set)? So that the result slide shows me 4 numbers, not just 1?

Any help will be appreciated please!
