We are trying to add a satisfaction survey at the end of some of our courses. We are using the SL Survey Slide Template. We have 4 Likert Scale questions on the one slide. When we load it into our LMS, we are only able to view the response to the first question, not the remaining 3. Are we missing something in the way we've built the slide?

It is up to the LMS in how they report on survey questions. I took a look in SCORM Cloud and the different items are getting reported, but the report is certainly harder to read. It's treated as one question with multiple responses/feedback:

Is there a way to adjust this output so that instead of "strongly_disagree" it says "1" (changing from verbal to numeric)?
