Do/Don't List

I'd like to build (or borrow if one already exists) a graded question that is a list of statements where the learner checks a box under "Do" or "Don't." I see how it would be possible to modify a True/False slide to do this for a single statement. I'd like to put 5 or more of these statements on a single slide. (See attached picture for clarification.)
Does that make sense? Any suggestions?

Hi Ike!
To do this, you'll need to choose the multiple response (in the graded question section) or the pick many (in the freeform section) option. Keep in mind, that learners will have to get all of the questions right in order for Storyline to recognize that the question is correct.

Adding on to Allison's response, you can use the multiple response question type to get exactly the view & functionality you posted by using a table and some radio buttons to control the selections for the statements. See instructions below:
