Do/Don't List

I'd like to build (or borrow if one already exists) a graded question that is a list of statements where the learner checks a box under "Do" or "Don't." I see how it would be possible to modify a True/False slide to do this for a single statement. I'd like to put 5 or more of these statements on a single slide. (See attached picture for clarification.)

Does that make sense? Any suggestions?

Adding on to Allison's response, you can use the multiple response question type to get exactly the view & functionality you posted by using a table and some radio buttons to control the selections for the statements.  See instructions below:

  1. Add the multiple response question and make sure ALL of your answers are marked as correct responses. (Both the DO statements and the DON'T statements).
  2. Add a table to the slide with 3 columns and as many rows as you need. Column 1 is labeled Statements, Column 2 is labeled Do, and column 3 is labeled Don't.  Add your questions to the the table rows. Be sure to position the table OVER the actual question statements and selection boxes so they can't be seen.
  3. Add radio buttons in all of the Do and Don't cells.  Be sure that the buttons in any row are grouped as a unique set (This allows only 1 button in each row to be selected).
  4. Add a trigger to each radio button to change the state of the appropriate question selection box to either selected or normal.
