Hi, 

Have a big kick-off meeting? The following document can help you gather important information about the project, your team mates, the learners and about the deliverable. I tried to keep it as concise and as simple as possible. 

 

How I have used the form:

1. I sent the form in an email as an attachment for the client to read, but not fill out. This helped the client prepare for the meeting. 
2. I completed the form with the client in person. This allowed me to clarify questions and answers.

Update:

I have shortened the project charter. I also changed the layout to portrait to fix some errors in the first document. This document is available in several different versions of WORD as well as a PDF.
