I'm my opinion there are two main ways to do this. The easy way is to set a quiz to have a print option and customise the report.html file that gets published in the output. You could add a logo and change the text using an HTML editor. This is ok, but is a bit rough around the edges (you cannot remove the URL from the bottom of the printed page from storyline).
The more complex way, but with a better result, would be to use javascript to make a PDF. You can pass variables like the user's name, the date, score from storyline onto the PDF. The worked example in the zip that Ryan posted will show you how good it can be.
