Hi Avril. I did a quick search of the community and couldn't find any such templates in our free Downloads, but hopefully, someone here has one they can share with you.
In the meantime, another community member shared a root cause analysis she created in Storyline in this older discussion. You may want to use the Contact Me link on her profile to see if she's willing to share her source file with you.
