including complex forms in the module

Hi all,
I'm looking for some inspiration. I will need to include a very complex form in my module (lots of boxes to tick and it's a one-page document). Ideally I would like to present the form in full on the screen which is only possible if I use a scrolling panel. But it's not ideal from a UX perspective as I will also need to include other features. Learners would need to click on certain areas of the form and read/complete an activity.
I only managed to find one similar example - applying for a US passport - which helped me explore other options. I wonder if you know any other similar modules on handling forms. Just wanted to see other examples and understand how others approached the task!

have you tried using an image of the page as a menu that zooms to different pages and they complete on the different pages you could pause the timeline of the zoom so it zooms in and out and looks like one page but is many.
There's a form in the example course I posted here a few months ago (MFD User Hazardous Waste Training.) Go to the course, click the Menu link in the upper right, and choose Accumulation Log, Part 1 to go directly to it, or you can just take the course up to that point since it appears fairly early on.
This form, like yours, is a printed 1-pager in reality. I ended up breaking it into two parts (top and bottom) in order to keep everything big enough to be easily read in Storyline.

Thanks Ray. I too decided to split the form in two (or even three) parts and show it full screen with some elements on top of it. I also liked the idea to use zoom - this can spice it up.

I liked the idea to use zoom. I tried it yesterday and was happy with the result. I wasn't sure what you meant by using the image as a menu - did you mean placing hotspots on certain areas of the form?
