Puzzle template

I'm looking for a 9 piece puzzle where either a puzzle piece will appear when the learner has answered correctly, or where the learner builds the puzzle (correct answer/s) from several pieces which would have correct and incorrect info.

I've attached an image of a nine piece puzzle that you can use for your project.
