Self-assessment in Storyline...

I have a project I'm working on and it seems like I remember seeing an example in the community, maybe posted by Jeanette, and wondered if anyone could help me locate it *if* it exists
Here's my project: I want to put together a self-assessment using Storyline where a user is presented with a number of leadership attributes with definitions for which they can give themselves a rating. At the end they are presented with the entire list of attributes along with the rating they have given themselves, and I would like to let them choose 2 or 3 of them to focus on. There will then be follow up information that outlines some next steps, etc. What would really be nice is if, after going through the self-assessment they could then print out something, but I'll cross that bridge when I get there.
Does anyone remember seeing a self-assessment type example in the community?

Hi Brian! Maybe you were thinking of this idea where you can display different result messages at the end of an assessment, based on how the learner performed?

Hi Brian,
now it is me looking for a process to implement some kind of self -assessment, where leaders are presented 1 question and 3 types of answers. The participants should distribute 5 points per question among the answers. Is it possible to put this together in storyline?
