Is there any chance you could share your .story file with us so we can see in detail? Even just the test portion would be great!
It could be a number of things without looking. I've noticed multiple choice options getting screwy in the past when you move or resize them in Slide View. Sometimes the choices will overlap as a result when publishing. I've found that allowing Storyline to position all of the Choices through Form View will often eliminate any positioning errors when populating Choice text.
If this is what you are running into it might be worth a try:
Go into the Form View for that question.
Delete all the text in the Choice column.
Pop back out into Slide View quick to see that the choices have all reset.
Then, go back into Form View and re-populate the text for your Choices.
