Using Articulate, PDF's with a LMS

Hello:

Can editable PDF's be incorporated into Articulate/Storyline and then loaded into a LMS (NetDimensions)?

Annually we send a form to our departments' 1,100+ employees to complete and return. To eliminate the tediousness of this process, I am wondering if the above scenario can be accomplished.  My greatest concern is the input of data, capturing and, reporting on that data on the back-end.

With NetDimensions we can load a Self-training Paper which would acknowledge receipt but, we need the employees to at first answer a yes/no question and if their answer is 'yes', then input additional data. 

Would a modified Quiz or Evaluation be a better route?
