This is a survey, which will be implemented for the 1st time, that measures a person's perception of their own competency, confidence & ability to complete specific tasks. This will become a standard tool of capability measurement every year; therefore, the metrics captured are critically important.
Essentially, it is a self-assessment tool for an individual to measure their level of capability & confidence against specified competency & performance benchmarks.

If you can it may be better to use a purpose built survey tool especially if the user will retake this a number of times. the reason I say this is that you are dependent on your LMSs reporting capabilities for the data you get out.
If you do want to use Storyline use the survey questions, avoid the Likert question unless you are using Articulate Online as an LMS and then use a results slide.

I have a similar question. I would like to make a survey that users fill in every day for a week and then they have to be able to see summary graph.
