Scenario with yes or no answer in table format

Hello All. I need to create a table interaction with 4 separate scenarios. The learner has to select whether its something they would do or would not do. I've tried a couple different ways to make it happen but it seems overly complicated.  I tried using states, and also radio buttons. Seems like it should be fairly simple and maybe I'm overthinking it. Wondering if anyone has a suggestion on the easiest way to program it. This is the simple draft I'm working with.
