Have you ever needed to have multiple text input fields on a single slide in a quiz? I've noticed a few other people asking for this kind of functionality and the answers, while creative, never met my need of having free-form answers.
Here's my solution to the challenge. It allows you to use as many text fields as you want. While it isn't as simple as just allowing multiple fields on a quiz slide it's pretty simple and easy to do.

It looks quite interesting but I am not able to quet the project file since storage netword are "evil" to websense.
I wanted to know if the solution can work with a graded question?

I just made it as simple as possible to focus on the problem of multiple text fields.

Thanks, Mike. I've been wanting to use multiple essay questions in an evaluation, and this fits my needs.
Now if I could just figure out how to capture the answers in a spreadsheet!

Yeah but it would require a little JavaScript programming on your part to output the quiz info into a comma-separated values file which any spreadsheet will be able to read.

Hi Martine,
A .story file is created with Storyline so, if you have Storyline installed, you should be able to open it.

Sorry to revive a slightly old thread but your solution looks perfect for what I need to allow multiple areas of text capture on one slide. My only problem is..shamefully..I cannot work out how you have done it :o(
Any chance you could give me (us) a brief run-through of the process for setting a slide like this up.

You, Sir, are a genius. Haven't had chance to try it on my existing resources as yet but it could well be the answer to the only major issue I have discovered in using Storyline and one I and colleagues have been trying to solve for weeks now.

So, I tried it in my story-file. I'm creating an evaluation form for a course and I want the participant to fill in some general info on the first slide.
It all looks good, but when I'm looking at the result, the info the user filled in on the first slide is not in the results.
Can you help me out with this? My storyfile is attached. If you try this out and then have a look at the result, you'll notice that the Short answer fields are empty although you filled them in.
