Thanks for posting your question here in the forums. You can't add a hot spot on top of text in a text block, but what you could potentially do here instead is hyperlink your text to a new lesson with additional information.
To hyperlink some text from a text block, simply select it and you'll see the editing window appears, with the ability to add a hyperlink.
If you really want to use hot spots, you'll probably want to look at the labeled graphic block (under Interactive in the All Blocks library) because that interaction does use clickable hotspots/markers.
