Collect detailed feedback in Rise, perhaps using Google Forms?

I have developed a technical course in Rise and want to collect detailed feedback in response to questions I will embed. Quizes won't work - there are no right answers and I want the learner's answer (not a score). Review won't work - I want to do this within the LMS and the questions are very structured and pointed. I read with great interest Steve Jone's post 'Embedding Web Content' and Rob Beer's responses where he is embedding survey questions using Poll Daddy.
My first impulse is to try to use Google Forms.
Anyone try this? Anyone with a solution for detailed feedback in response to questions on a Rise course? Thanks in advance.

I'm also trying the same thing. I've created a mock-up of how this might look, but it's still a little janky:

I have a similar wish. We have a "rank you skills" exercise we wish to create. Learners are presented with a list of skills. They are prompted to choose their skill level for each (ranked from 1 to 10). Ideally this data would be reported back to the LMS.
