I have a course that just has a test in it. If you fail the test and exit out of the course and then try to retake it - it only takes .you to the exit screen and won't allow you to retake the test. My settings are: Passing score 75%, Quiz retries : 2, Include in Lesson count on.
Publish settings: Track using quiz results (test), Reporting Pass/Failed.
It sounds like revisiting the course in the LMS is not allowing you to retry the quiz. Is this happening after all quiz retries are exhausted? If so, this is expected behavior. You can allow unlimited quiz retries if you'd like them to continue trying until they pass.
Otherwise, it will be up to your LMS whether you can reset courses for learners manually, or if it will launch a "completed" course in review mode only. Review mode is generally used by the LMS to prevent learners from getting lower scores on previously completed content.
You also mentioned the failed vs. incomplete status. Depending on the SCORM option you chose, your LMS will need to interpret the data that your course sends upon completion. Check out this article for more details! If you have it set up for pass/fail, your LMS may interpret the fail to also mean "complete."
