I cannot see the Add Question button to add a 3rd question. I have tried it in Google Chrome and IE. My resolution is at 1280*1024 which are at the recommended for my display.
I was using Chrome but the problem was the Add button did not appear on the left side panel. What I found was a message at the bottom recommending which browsers were supported. When I clicked the image on the banner it made the Add Question button appear.

In the quiz editing panel, there should be an option to Add Question at the bottom:

Sorry it's still not working for you! You'll want to avoid using Internet Explorer since it isn't a supported browser for authoring Rise content. Chrome should work, though!
Here's a quick screen recording of my testing in Chrome v.61. Are you seeing something different than that? If so, could you record a quick Peek of what you see on your end?
