You could make a web object that uses a tool like survey gizmo or survey monkey. That way you could easily download the results for editing, reporting etc. I'm a big fan of Surveygizmo.com and they both have free account options that work great.
Google docs could be used this same way without needing the embed option too.
@Phil Ouch! I feel your pain on that one. That's why I often publish the web object to be a local blank html page with a or embed code so that I can update stuff like that without having to republish.

Hmm, I might be blanking on an easier method, but what about using something like Google Docs to capture your student's essay responses? They're easily embedded and can be saved as a document...

Google docs can only now be embedded if you open a new browser window. Google now prevents its sites loading in an iframe

I did say you could open it in a new window, just having a whinge because I built this into 90 scorms and they broke it the day i launched
How about Windows Online? They got anything like a form that can collect data.
