Self-Assessment Support (RISE)

I'm hoping to build a self-assessment in RISE where participants can assess their leadership skills. Since there won't be right or wrong answers, it looks like my only option is to use the checkbox block and have them select from a range of statements (Strongly Disagree to Strongly Agree). I'd want to them give them feedback based on their selections. Does anyone know how I can do this? Even better, does anyone have an example?

My hope was to aggregate the scores and give them a development statement for each leadership topic but I think I'll be limited there.
