Adding multiple text data entry fields as questions on one slide (not a freeform question) in Storyline 3

Would anyone be able to advise if it is possible to add 3 questions to slide content using data entry fields where learners type their answers and to select yes/no.

Then based on their answer/selection we response with feedback next to the learners answer.

The effect we would like to get is that of having an interactive conversation with the learner.

 

See attached example of one of the images that l need design on one slide.

 

If this is not possible, does anyone know of an alternative solution.
