I think my trigger is set up correctly (see the highlighted trigger in my screenshot), but I am getting 1 subtracted on the first intersection and then 2 subtracted each time after wards.
At a glance the trigger setup looks correct. Just for kicks, I would delete the Subtract 1 trigger and add a new trigger that subtracts 1 when the audio file completes playing. That way you could eliminate the possibility that the intersect events are somehow fired incorrectly.
Since my suggestion worked I would still try to figure out why the original trigger didn't execute as expected. Maybe your car bitmaps have some transparent bits that overlap the frog in two places at the same time?

Great idea, Michael. Thanks! It shouldn't be necessary but it got the job done.
While I was at it, I tweaked the timing and number of lives to make it a bit tougher. If anyone wants to play, the link is the same as before.
