Create a certificate with name and date as a PDF ?

Is it possible in Storyline to create a self-designed certificate as a PDF at the end of the course, where the user is asked for his name beforehand and then the current date is also entered?

In my case, the user doesn't need to have passed a test or anything. It is simply a designed page in which the username and the date are entered.
