SCORM 2004 LMS reporting

Morning all, I am new to Articulate and the LMS world. We are working with Kallidus LMS, and I have designed a Rise 360 course for our logistics and Supply Chain induction. As well as the information sections, I want to capture confirmation that individuals have seen or completed activities linked to their induction, more of a form than a quiz.

I have built these forms/quizzes in Articulate Storyline and then pulled these through to a block in Rise 360; I am then exporting to SCORM 2004 with reporting set to the specific block: from reading bits on this forum I now know this isn't going to give me what I want. 

Does anyone know of a solution? I used Google Sheets previously, but I want to capture this in the LMS quickly. The questions are either 'yes' or 'no' but are not actual questions more of a confirmation that something has happened. 

Any support would be greatly appreciated.
