Second attempts shows the answer is incorrect but the learner choose the right answer

Hi folk,

I met a problem that a knowledge check allows the learner have two attempts. If the learner has a second try and clicks the correct answer, the quiz result shows: you are wrong.  I checked the quiz form, and there is no problem with it. 

 

Any suggestion to fix this problem would be appreciated.

    If you will attach your .story file (or at least that slide) here, someone can take a look at it and give you some ideas.

      Thank you, Walt. I've attached the slide. The correct answer is 'No'. When I choose the wrong answer, SL asked me to try again. Even if I choose the 'No', the result still shows I am wrong.

    to check your problem, we need the .story file of this one slide, not the screenshot of the slide
