Quizmaker provides wrong feedback to a question

I have a number of packages uploaded to our LMS system.  Last week I had reports that 2 packages had provided incorrect feedback on a question. 

Package 1 question was a simple true/false no shuffle, failed a user who had selected the correct response.

Package 2 was a picture question with shuffle answers on, user should click on the correct picture, again when checking the results the user had selected the correct response but system said it was incorrect. 

I've checked both packages and the answers in Quizmaker were correct.  I have no idea what's happened, I've taken the tests myself this morning without any problems. 

Both packages have been uploaded for months without issue.  Any advise would be most welcome.
