Conditions for a multiple choice quiz question

I have a quiz question with three triggers and two layers.
If someone chooses any of two checkboxes I specify, they should receive the incorrect pop up (layer).
If someone doesn't choose any checkbox, they should receive the incorrect pop up (layer).
If someone chooses three of the five checkboxes that are correct, they should get the correct pop up (layer).
Although the incorrect displays as it should, if someone selects the three correct answers, the incorrect layer also displays. I've triple-checked my conditions...seems pretty straight forward...but it isn't working.
Any ideas?

Hi Carrie! Can you share your .story file here for folks to have a look? Maybe there is a slight adjustment that a second eye can spot!
Sure. Being brand new to Storyline, can you please tell me what specific file you need. I don't see a .story file anywhere.

I don't know if this matters, but I'm using Articulate 360. I found the M:y Articulate Projects folder in my Documents folder. It contains a few subfolders and files. There is not one that has a .story extension. I did a search - nothing.
Apologies!! I never actually saved the file - published it/previewed it/saved the project, but..
The first attachment you can completely disregard. It's the second that has the checkbox issue. Thanks for your patience!
No problem! Your Storyline courses will save in a defaulted location, usually on your C: drive in your Documents folder > My Articulate Projects folder. It will be "filename.story" and you can attach it in the discussion! You'll have to do it via the website, however, because attachments don't come through when replying by email. Your contact information does, though, so you can edit that out of your last post, if you wish!

If this is the slide you are referring to - there are no correct selections in the form view
I have SL360 and I believe I upgraded. However, I did use an old file and modified it to create this one.
I did select the correct answers in Form view, and the incorrect ones. I also added the triggers to display the layer that should display. I even believe that it worked at one time but I moved the slide around a bit. At this point, I think I'm going to recreate the quiz slide - or delete it and use another type of quiz slide.
