Unless I have to use one of the built-in hotspot question types, I always use transparent shapes, rather than hotspots. To me, transparent shapes are more flexible because they can have states.
That's what I thought. Though, with freeform quizzes, you can only have one hotspot in the interaction. That's why I like using transparent objects to create the same effect. At least this way you can use the "Pick Many" quiz option for more than one correct answer.

I would say the only advantage would be the pre-programming of the hotspot, and the freeform ability.

Also one advantage of hot-spot is we can remove hand cursor from the clickable.
