Multiple fill in the blanks?

Hello,
I’m trying to figure out the best way to create an exercise like the one below without having to create one slide per sentence. Would you use fill in the blanks multiple times? Short answers? Freeform text entries?
Fill in the blanks with the simple past of the verbs between parenthesis.
1. I_____in New York yesterday. (to be) 5. I _____ the book this morning. (to finish)
2. She_____ to the store. (to go) 6. We _____ well in that restaurant. (to eat)
3. We_____many presents. (to buy) 7. The children_____the window. (to break)
4. They_____the truth. (to tell) 8. We_____football in the park. (to play)
