I need to take two options in the form of a tick if possible for many questions on a single page. Not sure the best way, any ideas? These options could be blank, either or both.

the ticks or blanks will be under the required and achieved.
Hi Yusuf! Do you need to report these selections to an LMS or just use them within the course for feedback and/or interactivity?
One way you could do this is by using a variable for each answer option. For example, each check box might have an associated True/False variable with a default value of False. You could add a trigger to set the value to true for any check boxes that are selected.
Hi Yusuf, I'm nor sure how the 'I will need to capture free text.' part figures into you original question about multiple questions on one page. Is this what you want? This is a sample with five questions on one page, which can easily be extended to 10 (or more). This was done with the built-in checkboxes, variables and some simple triggers. Some info on variables is here.

Hi Yusuf, see attached the source file. Yes, you could adapt the triggers in my sample to check which of the option(s) was clicked in each question.
