I have a 10 question quiz that requires 100% in order to complete. When I'm testing, my quiz completes even when I fail the quiz. I'm not sure if it's a Storyline issue or a setting in my LMS but I'm at a bit of a loss. I'm using Storyline 3. Thanks for any help the hive mind can provide.

As you mentioned, you are using Passed/Failed as your reporting option. That means that as long as the user completes the course, it will be marked as complete, but with a success status of either Passed or Failed. See below:
Changing the reporting to either Completed/Incomplete (with no success status) or Passed/Incomplete should show Incomplete when the user fails the quiz.
We are having similar issue, using Passed/Incomplete but LMS is still marking the course complete as we attempt any question, and exit without passing the quiz.

I would upload the published content to SCORMCloud and test. If it works there, then it's likely a setting in your LMS.
