I'm running ver 12: 1705.20 and update when prompted.
Again, I copied the "problem child" into a new.story file and it behaved there. So, I deleted the "problem child" in the initial project and copied the now "obedient child" into the initial family....viola harmony!

Just to let you know one fix was to copy/paste slide into a new project. The test was successful. I was able to access Form View.
Then, I copy/paste back to original project and it worked there as well.
Would still like to know what cause the issue or if it was just a glitch.

Without seeing the file that caused the problem, it sounds like a glitch.
When you run into issues like that it's always a good practice to check the following:
