Using a quiz in self assessment

Hi All
I am looking for advice on how to create a Situational Leadership assessment that we currently use in paper into a Storyline 2 content piece. the way the self assessment works is there are 12 questions and depending on how the responds to a question it relates back to a leadership preference. Basically the need to tally up the number of responses to the corresponding TYPE to gauge preference.
Please see the attachment for the response patterns.
I have tried a few different ways using grade by choice etc, but I could not produce the desired result.
If anyone has any ideas or recommendations, it would be much appreciated.

how is this scored 'on paper'. If I selected all 'A's how would it work out?

Basically, it is a tally system. If you take a look at the attachment it may be clearer. If I select the majority of my responses under type A, I would be Type A. If 6 responses fall under Type A and 6 under Type B, I would be borderline A and B.
