Me too. I was looking for an add button like you see on the quiz blocks. Thanks for asking, it wasn't a silly question. It was really frustrating me trying to work it out! :-)
Also weighing in, here, that this workflow is not even remotely intuitive, especially since to add more buttons in a button block, or to add more items in a list, the sidebar menu provides an "Add item" option (which serves as an instruction). I, like others for years here, (apparently) was looking for this conventional 'add' feature. You could simply add an instruction in the sidebar in the 'Edit' window below the intial 2 markers provided (i.e., in the same location as the "Add item" instruction in other places) to the effect of, "Click on image to add more labels". Since people will see that instruction when the 'plus sign' icon is active, it should be more readily evident that that's what one is supposed to do. As it is, there are no instructions at all for adding a new label.
Currently, marker styles don't go above the number 9, but we're tracking requests for this feature. We'll add you to the list of users requesting this, and we'll update this thread on any updates that can help!
From all the comments I"ve read here, it seems this feature is essential for interactive images as is the need to be able to make the markers larger. When might that happen?

Yes it was helpful. I agree that it was easy, but not intuitive because other blocks use the "Add" button
I already knew this; my question is if you can actually make the markers larger, and how to make double digits as the numbers only go from 0-9?
