Here's an example of a video quiz I built in Quizmaker: http://elearningdesigner.com/projects/2011/spot/video-quiz/presentation.html
You can jump to the quiz if you go to Menu > Video Quiz. I'll update the project to Quizmaker 360 and remove the other slides so it's easier to find the quiz.
