Pulling a learner's name in to a certificate

Hi, 

I need my courses to produce a certificate at the end that include the learner's name and a date stamp (the course name would be a nice-to-have). Can anyone tell me how to do this? I've been reading through these articles and haven't found anything that is very helpful.
