Working with your solution I discover that all elements (text placeholders and the button) must be left as-is, other than formatting for color, size, etc.
Although workable, this is a poor design because it implies a level of flexibility that is just not there, And allows--if not encourages--going down the frustrating path of no return that prompted me to begin this string.
The way this area of functionality is positioned is clearly that what you see in the feedback master is what you get in the slide, and that is simply not the case. I hope this discussion can save others from the frustration we have faced.
