This thread is a bit dated, but if you do not hear back from Tom soon, you are certainly welcome to reach out to the user directly via the 'Contact Me' option on the user profile.
Utilizing graded questions or even button sets would help you accomplish the task you mentioned though.
Looks like Tom wanted the immediate feedback and allowed the user to select multiple.
