Knowledge Check options cannot be selected

I have a course with a variety of multiple choice and multi-select knowledge check questions. When I preview individual slides, they work fine. When I preview scenes (with 2-3 knowledge checks in them), they work fine. However, when I preview the entire project or publish it to Review 360, some of the knowledge checks are no longer functional (the user cannot click the options with either mouse or keyboard). Some QA testers have also shared that refreshing the Review 360 page resolved the issue (the knowledge checks worked correctly on a second try).

I don't have any conditional triggers or variables in place that would restrict the options so I'm not sure what could be causing it. The only solution I can think of right now is to take the one or two that do work consistently and copy/paste them to use as a baseline to re-make the ones that don't work. But I'm hesitant to spend time doing that without knowing if it will resolve it (since I haven't determined any key differences between the ones that work and the ones that don't).
