Need Help - Question Bank

I created a assessment with 36 questions which I moved into a question bank to randomize 25.  My project shows the assessment instructions with a next button to the question bank and a results slide but when I try to publish for review it shows an ellipse... and never publishes.  I am also unable to preview the Assessment. HELP!
