Questions in Bank not Shuffling
I have a build with four question banks, each one set to be drawn from randomly. It seems the first draw is random. However, each draw after that is the same question, over and over.

I did a quick test on my end to test the randomizer functionality and I can confirm that the questions appeared randomly in my test course. You can check out this link to see if you get the same behavior. You'll want to make sure that you have this option enabled for your question banks:
If the behavior is specific to your course, would you be willing to share a copy of your project file here or in private by opening a support case so we can take a closer look at what's happening? We'll delete it when we're done testing!
I've attempted to attach the .story file to a case, but I keep getting an error. Is there another way I can share it?
